not a dental insurance plan.
Exclusions apply:
* Each plan is valid for 12 months
* Membership is for existing patients only.
* Teeth Whitening, veneers, and Invisalign are considered cosmetic treatments and are not a covered benefit.
* Annual premium is non-refundable and non-transferable
* Coverages can change when plans are updated annually
* Cannot be combined with any insurance plans
